The Rossford Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms collects, organizes, and updates our athletic participation forms easily online. FinalForms will save data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice.
You will have to review your information once per school year to verify information is current and sign the forms. This online form is very user friendly and will save time and paper compared to the large old packets. The program will email you notifications for when your child’s physical examination expires.
The online forms include a drug testing form, and while junior high school students are asked to complete the form to finish FinalForms, only high school extra-curricular students are tested.
